My after visit notes say I consented to Dax Copilot. I specifically declined and did not consent. This is a violation of trust. I'm calling soon to determine if this was a mistake in the notes. Otherwise, I will not be returning to this provider.

I just want to say Nurse Practitioner Autumn is great.

I was able to see this provider the day I called at a convenient time. I received care plus a prescription. I also had a referral to a local dermatologist and saw that practice the same week. All people were wonderful to me and understanding. The nurse was very thorough and patient. thanks

This visit was two fold it felt with a gout flare up and post hospital stay for a GI bleed Both issues were very well addressed with additional follow up testing. Autumn spent additional time with me learning more about my health history she was very attentive

This was a checkup appointment but the provider gave me information about my conditions that other providers have never told me about so it was well worth the appointment time and I appreciated it.

I had a very good semi-annual check-up with my family physician and reviewed the test results which were out on the mychart website. the physician reviewed the results earlier and provided comments to each result on the website which I reviewed before I had my appointment, which was very helpful. She gave me some suggestions to follow up with to further improve some of my health issues, which I will do.
