The visit summary indicates that current issues are all under control as if I have no problems. I am still having a lot of back pain and I am currently seeing a pain management specialist to try to help. The summary states no back pain. I stated I still am having some depression and anxiety, however the summary indicates that everything is all better and no issues. I don't know that I agree with all of the summary statements. Yes, I am trying to manage my stress, anxiety and depression better but it is by no means gone.

Dr Aziz is a wonderful physician. She listens with intent and compassion. Would recommend her to any of my family and friends.

I had to make multiple calls to get someone for an appointment. There was no problem after I got through. This never happened in all of the years I went to the facility in Beavercreek. I think there should be a different telephone number for non-pediatric patients. I believe that would solve the problem.

The nurse assistant did not address me by name, instead was using words like girl boss? Respectfully I would be prefer to address by name. I am a senior woman, please train youth correctly. The provider did not request refill for my meds, and did not supply blood work request.

Professional and attentive to my questions. Went the extra mile to explain the results from recent cardiac tests, which despite my repeated requests for a f/u appointment for interpretation and possible medical intervention, appear to me to have been "ignored" by the ordering cardiologist. I so appreciate Dr. Aziz!

This was my first visit meeting Dr. Aziz. I liked her very much. I appreciated her candor and how she approached me as a patient. The building was in need of update. I was uncomfortable meeting with my new doctor surrounded by pediatric patients. The office space needs to be separated for dedicated adult and child rooms.

Dr Aziz is vey kind, and truly cares about her patients.

The nurse who took us to the room was so friendly. Dr. Aziz always explains tests results and reduces my stress over medical terminology in My Chart. Wonderful team.

I would of liked to have had a scan ordered with my prior history. But was only prescribed antibiotics and referenced to get a check up for colonosocpy. Other than that I fully enjoy Dr Aziz.

The provider made my experience easy. I was brought to a room in a timely manner and my doctor was extremely caring towards my current situation. I felt very heard and cared about.

I love Dr Aziz. In all my life and medical history (which is extensive) I have never had a more caring and compassionate and competent doctor. I recommend all my friends and family to her. Didn't like being in a pediatric exam room and having to be around a bunch of kids. Dr. Aziz apparently is sharing space with pediatricians and I hate that. Don't know if those kids were sick or wellness checks but internal physicians should have an office away from kids especially for the elderly. May not return to that office.

It has been my experience twice in contacting this new office where my provider has transferred that the reception staff did not respond to telephone calls within a reasonable period of time. The first time was when I checking if an earlier appointment made at the former office was going to be kept by the provider. The provider's name was not even given as an option in order to contact the right receptionist group. I was obliged to call the former office to learn how to get to the right group. Then when trying to keep my appointment, I did not realize that the new office address does not follow numerical order. I called then this second time with the provider's name still not being listed as an option, but followed the previously given advice. I received no useful response for my particular dilemma, simply a series of other numbers to punch for options that I currently did not need. Fortunately, I have another provider located on that same street and went there seeking directions. (My GPS also had problems with locating that address.) That reception staff kindly assisted me and I found the office building. I finally received a return call and text about 10 minutes after I had arrived and was involved in the check in process. Please consider updating the provider telephone contact information to include the provider. Thank you!

The medical assistant was super nice and engaging. After seeing Dr Aziz in the new office in Beavercreek, the office in Centerville felt old and outdated. The exam rooms felt super small I hope that practice is slated for an update soon.

Just two negatives: (1) I did not like sharing the facility with pediatrics. Did not feel safe in the waiting room with sick, crying children. I hope that changes in time. (2) Also, they forgot to order the prescription refills. I'll make sure to remind them next time.

Dr. Aziz is an excellent Internal Medicine physician with a kind and caring bedside manner. She answered all of my questions and concerns and I didn't feel rushed during my appointment. I am so glad that she is my physician!
